An investigation by Special Agents with the Medicaid Fraud Control Unit of the Tennessee Bureau of Investigation has resulted in the arrest of a Livingston physical therapist on forgery charges.
At the request of 13th District Attorney General Bryant Dunaway, on October 6, 2017, TBI Agents began investigating Dustin Rich on allegations of TennCare fraud. At that time, Rich was employed by a Clay County hospital as a physical therapist. During the course of the investigation, Agents developed information that Rich created false documents and forged the signatures of the patient and/or guardian, which resulted in TennCare being fraudulently billed for those services.
